Care Planning at Friary Care in Weymouth
Anyone who uses care services, must be provided with services that they have been part of choosing, that meet their needs and are based on the best research and standards. In this short article we will try to explain what is a Care Plan.
Care plans are the way we plan and agree how someone’s health and social needs can be met, and how good health and wellbeing can be supported. They are the most important document that professionals, service users and their families share, and are the means of communicating an approach, a service, or a treatment between all those involved. We believe that this process is so important that it needs proper support and training to help people develop skills and experience in working in partnership to agree a plan.
A care plan is…
- The written record of a plan of action negotiated with the person to meet their health and social needs
 - Something which sets out who is doing what, when and why
 - A tool to support the safety of the service user and others
 - A plan which can be used and understood in a crisis by service users, their families and carers and other agencies, as well as colleagues

 - Shared effectively with those who are part of it
 
Care planning is unique to everyone who makes their home at Friary Care and begins from the first moment they agree to come to Friary Care and is updated constantly to accommodate changes in need and wellbeing. We make every individual’s care plan available to them and to their relatives if they give permission.
The care plan is a file held in the office and contains:-
- an ID photo
 - next kin details
 - future wishes
 - mental capacity act wishes
 - weight
 - height
 
Individual care plans are about the person care needs such as washing, dressing, communication medication, promoting independence, nutrition.
Care plan cover specific needs communicate every detail to staff so they know what care is required.
The care plan file is data protect and is locked away, except when being used for reference by members of Friary Care staff, other health professional and any other person that has health and welfare permission from the client, in the form of power of attorney.
